To make your visit as efficient and effective as possible, we ask that you have the following information ready for your evaluation:

• Full body picture to upload
• Photo ID
This enables the provider to prepare for your consultation and tailor our services to best meet your needs. Your information is confidential and securely handled according to the highest standards of privacy.

Weight Loss
Telehealth visits cost $50 per visit to be collected by the pharmacy upon medication pick up. Once the visit is complete, your prescription will be sent to the pharmacy. We will contact you to discuss pricing and options for receiving your prescription. You may pick up your prescription at the pharmacy or have it shipped to you.
Shipping is available to Georgia and South Carolina. Shipped prescriptions are sent overnight via UPS and shipping costs $25. The prescription is a refrigerated product and must be sent overnight. Prescriptions are transported in a cooler box with ice packs and a temperature sensor.
Semaglutide
